Be fully vaccinated

Be tested for COVID-19 weekly, at a minimum

Mask Protocol
- Effective August 30, 2021, the State of Illinois is requiring masks for all indoor public places
- Public places include church services, ministry events and/or sporting events
Vaccination Information
You are considered fully vaccinated:
- 2 weeks after the second dose of Pfizer-BioNTech or Moderna OR
- 2-weeks after the single shot of Johnson & Johnson
Definitions
- Close Contact – Within 6 feet of someone for a cumulative total of 15 minutes or more over a 24-hour period (unless you have been fully vaccinated)
- Direct Contact – Being in direct contact with secretions (i.e. coughed/sneezed on, kissing, sharing utensils, etc.)
Symptoms
may appear 2-14 days after exposure
- Fever of 100° Fahrenheit or more
- Persistent cough
- Pain/Pressure in the chest, body aches, shortness of breath or trouble breathing
- Headache, Nausea/vomiting, Diarrhea, Sore throat, Bluish lips/face
- Loss of taste/smell, feeling confused
- This list is NOT all-inclusive so please consult your healthcare provider before coming to work if you have other concerning symptoms

- Avoid close contact with people
- Cover your cough or sneeze
- Avoid touching your eyes, nose and mouth
- Wear PPE/Face Mask and practice social distancing
- Clean and disinfect objects and surfaces
- Stay Home when you are sick
- Wash your hands for at least 20 seconds. Participate in daily health checks/screenings
